1st-4th grade: Practice will be held in Manson at MACC Fields, Game times and locations TBD. 

• $45 for MACC Members or $55 for non-MACC Members – There is an additional fee of $3.00 for those signing up on-line.

Equipment Provided: 

• •  Jersey
  • Softshell Helmets –  3rd and 4th grade. Submit head measurements on form below. (Measure in inches, around head at forehead.)
    

Equipment Not Provided:

• •  Mouthguard
  • Cleats

BY ENROLLING ELECTRONICALLY WITH THE FORM BELOW YOU AGREE TO THE FOLLOWING TERMS:

WAIVER OF LIABILITY: I, the parent/guardian of the registrant agree that I and the registrant will abide by all applicable rules of this enrolled sport and will display good sportsmanship. I recognize the possibility of physical injury and hereby release the MAC Center, all coaches, assistants and any other persons associated with this program and the City of Manson from any and all liability related to participation in this program, including but not limited to injuries sustained during participation as well as transportation to and from practices and games are part of this program.

CONSENT FOR MEDICAL TREATMENT: As the parent or guardian of the registrant, I hereby give consent for emergency medical care prescribed by the primary healthcare provider. This care may be given under whatever conditions are necessary to preserve life, limb or well-being of my dependent.

PHOTO CONSENT: I give my permission to the Youth Cougar Club to use images/videos of these participants taken during the program to be used for future promotional material without compensation.
